IDA P- 1.TAB PANTOP 40MG PO/OD 2.INJ BUSCAPAN 1AMP /IV /SOS 3.PLAN FOR BLOOD TRANSFUSION 4. SYP CREMAFFIN 15ML PO/HS 2) 60/f  S: no fresh complaints . vomitings-subsided  loose stools-subsided  pain in abdomen- subsiding   O:  Dry tongue+.  Pallor+. Temp: afebrile PR :84 bpm BP:100/70 mmhg RR: 18cpm P/A: soft, tenderness noted in epigastric region. CVS: S1,S2  heard, no murmurs  RS: BAE+, NVBS heard. CNS: NAD. A-    Acute gastroenteritis.   P- 1.IV FLUIDS 2NS, 1 DNS, 1 RL @ 100ml/hr. 2.INJ. PANTOP 40 mg /po/OD.. 3. INJ.ZOFER 4 mg/IV/TID  4. TAB. SPOROLAC -DS PO/TID. 5. Prof 67 year old man presented with the complaints of Dyspnea on exertion since 3 months Giddiness on and off since 1 month Patient works as a farmer at a local village. He is a father of 2 children. He has consumed alcohol mainly whiskey everyday the last 35 years. Since 1/2 an year, he has been only drinking whiskey on rare occasions. He also has been a cigarette Smoker for the last 35 years with around 20 cigarettes per day. He stopped smoking half year ago. On routine checkup, he was told to be a diabetic 18 years back and also was told to be a hypertensive 2 years back. Since then he has been using medications religiously. 2 years back, he started noticing blood in his stools on and off for which he visited our hospital, KIMS. He was diagnosed with anemia back then and also received 2 blood transfusion

MRI brain revealed that granulomatous lesion in right temporal region. Further evaluation revealed a history of anxiety and depression.
